What happened at the end of This Is Us Season 6?
Surprisingly, the first scene of the episode is not of the Pearson family but of another family that is in a car accident. One of the kids in the backseat, Marcus, wasn’t wearing his seatbelt, so he had to go to the hospital after the accident. His worried family is waiting in the waiting room. When the boy’s father went to get some coffee, he ran into Jack. Jack is there right after the fire, and the discussion they have is the last thing they do together before he dies. The same doctor who took care of Jack was also a big part of keeping Marcus from dying.

Fogelman said that the proposed This Is Us movie will be about an alternate timeline in which Jack survived the fire.
